We’re here to connect with the right providers to assist in building your capacity to manage supports, and overcome barriers that may impact your progress. We also work in collaboration with your providers to ensure your care team is working in unison to provide the best outcomes possible for you. We are here to take away all the administrative related stress, such as budget allocations and tracking so you can focus on engaging in sessions with your teams.
For participants with more complex needs, our Specialist Support Coordination (Level 3) offers higher-level support coordination delivered by experienced practitioners who can navigate multi-disciplinary teams, crisis situations, and complex service environments with compassion and expertise.
